Low fertility, high stakes: demographic and economic implications of Malaysia’s fertility decline and the case for a National Fertility Insurance Policy

Abstract: Malaysia’s fertility rate has fallen to 1.6 births per woman (2024), below replacement level for more than a decade. This decline is driven by delayed marriage, economic pressures, and limited financial support for infertility care. Infertility affects 1 in 6 adults globally, yet treatment in Malaysia remains largely self-funded, leading to inequalities in access and increased emotional and economic burden. Understanding these barriers is critical to strengthen national demographic resilience. Conclusion, Malaysia’s fertility decline is a multidimensional issue requiring urgent clinical, policy, and economic intervention. The National Fertility Insurance (NFI) framework presents a strategic, sustainable, equitable policy mechanism to improve reproductive health access, sustain population growth, and strengthen socioeconomic resilience. Strategic investment in fertility care can safeguard Malaysia’s socioeconomic future.

Disaster preparedness towards community resilience: curriculum development of a flood disaster training module for communities

Abstract: Floods are among Malaysia’s most frequent and devastating disasters, affecting more than 4.8 million people annually. Rapid urbanisation, deforestation, and climate change have increased flood risk and severity. Community preparedness remains in adequate, particularly in rural and low-income areas. Strengthening community resilience through structured training is crucial to mitigating the impact of disasters. Conclusion, The Flood Disaster Preparedness Training Module effectively improved community knowledge, confidence, and coordination in flood response. Its participatory design ensures local relevance and sustainability. Intergration into national training frameworks and future digital expansion will strengthen Malaysia’s disaster resilience, empowering commnunities to better prepare for and recover from flood events.

Abstract: In conjunction with the 2025 World Population Day Celebration themed 'Empowering Young People to Shape the Families They Want in a Just and Hopeful World”, the National Population and Family Development Board (NPFDB) conducted an online public opinion survey from July 1 to August 30, 2025, to understand the aspirations, obstacles, and hopes of the community regarding the desire to have children. A total of 3,625 respondents, consisting of Malaysian citizens aged 18 and above, participated in this survey, including 2,627 married respondents (men: 932; women: 1,695) and 988 single respondents (men: 380; women: 618).

Abstract: The phenomenon of depopulation will become a new challenge for Malaysia in 2060. The population is projected to peak in 2059 at an estimated 42.38 million people. The total fertility rate (TFR) will decline to 1.4 by 2060. The average age of mothers at first live birth will increase (2023: 28 years). Median age at marriage: men: 28.0 years, women: 27.0 years in 2023. Household size is decreasing to 3.7 (2024), 5.2 (1980). Life expectancy is increasing, men: 77.2, women: 81.5. The evolution of national socio-economic strengthening focuses on restructuring the economy for a quality life for the people. As the leader in official statistics, Department of Statistics Malaysia (DOSM) will begin to release aggregate/statistical data at granular levels regularly. Trusted figures and data are the lifeblood of growth and fairer distribution.

The impact of covid-19 lockdown on family planning services in Selangor: pattern and geo-demographic profiles

Abstract: The Movement Control Order (MCO) or lockdown implemented in Malaysia as a measure to prevent the spread of COVID-19, affects all sectors including education, business, health, and more. Family planning services are among those affected by these restrictions. Family planning is important in improving family well-being and is one of the main objectives of the Sustainable Development Goals (SDG), 2015-2030, and the Twelfth Malaysia Plan (RMK-12). Therefore, this study aims to identify the trend and distribution of contraceptive use in Selangor, before, during, and after the MCO. In this study, the geo-demographic profile of contraceptive users was also investigated. The methodology of this study involves the use of descriptive analysis and the K-means clustering method in determining the client's geo-demographic profile. The data used in this study is the new contraceptive user for five Klinik Nur Sejahtera (KNS) in Selangor, for the years 2019 and 2020. The results of the analysis found that the use of contraceptives in Selangor showed a fluctuating trend before and after the MCO, with service interruptions during the MCO in 2020. Findings from the contraceptive user distribution mapping showed that more than 50 percent of KNS clients were within a 20 km radius according to the five districts for the year 2020. The analysis of the original pattern on the map shows the need for a clinic to be built in the northern part of Selangor. In addition, the geo-demographic profile findings provide five clusters of KNS client characteristics for the year 2020. In conclusion, the results of this study are significant and can be used by the authorities, especially family planning service providers to identify areas that need further scrutiny such as the allocation of resources, training, and awareness campaigns. The geo-demographic profiling of clients can provide authorities with a deeper understanding of their client's characteristics thereby enabling them to improve family planning services.

Abstract: The overall IKK score for Malaysia is 7.72 (2019) while the score for 2022 is 7.93. The overall score of IKK for the State of Selangor is 7.99 in 2019 and the overall score of IKK in 2022 has decreased to 7.87.

Abstract: In conjunction with the 2024 National Mother's Day celebration, the Ministry of Women, Family and Community Development (KPWKM) through the National Population and Family Development Board (LPPKN) has conducted an online public opinion poll from 12 to 31 May 2024. In addition to finding out the issues and the current challenge of motherhood in managing the family and raising children, the focus of this study is to find out the level of mental well-being of mothers, and to know the extent of the important role and involvement of a mother in family institutions. With the theme "Terima KASIH Ibu", a total of 3,988 respondents consisting of mothers of Malaysian citizens aged 18 and above who have children were involved in this survey. The results of this study are expected to be the basis for the implementation of appropriate intervention programs towards the strengthening of family institutions.

In addition to finding out the issues and the current challenges of fatherhood in managing the family and raising children, the focus of this study is to find out the level of mental well-being of fathers, and to know the extent of the important role and involvement of a father in family institutions. With the theme "Terima KASIH Ayah", a total of 1,238 respondents consisting of fathers of Malaysian citizens aged 18 and above who have children were involved in this survey. The results of this study are expected to be the basis for the implementation of appropriate intervention programs towards the strengthening of family institutions.
